We value your time and strive to make each appointment productive and efficient. To ensure we maximize the allotted time for each patient, we request that you fill out all the necessary forms prior to your scheduled assessment/physio session. This allows us to understand you and provide timely and professional care.

Each form will take approximately 3 to 5 minutes to complete. Please check and ensure all details are accurate and up-to-date. Once you’re done, print out the completed forms and bring them with you during your appointment. You may also send them to us via email at hi@biokineticpt.com.

HIPAA Compliance

Protecting your privacy has always been our top priority. As your healthcare provider, we strictly adhere to the Health Insurance Portability and Accountability Act (HIPAA) to ensure confidentiality when handling protected health information, which includes:

Common identifiers, such as name, address, birth date, and SSN
Past, present, or future physical or mental health condition/s
Type of care provided
Past, present, or future payment transactions for physiotherapy services

Only authorized members of BIOKINETIC Physical Therapy have access to all patient records. As an added security measure, we have also encrypted our online forms to protect your data from cybersecurity threats. We assure you that your patient information is used solely for treatment and payment-related purposes.

Notice of Privacy Practices

We provide every patient with a Notice of Privacy Practices, which explains how your information is managed and shared in accordance with HIPAA guidelines. Please download the Notice of Privacy Practices form and bring a signed copy during your appointment. You may also email it to us at hi@biokineticpt.com prior to your assessment.

Out-of-Network Physiotherapy Services

BIOKINETIC Physical Therapy and its provider, Oliver Jay Realino, is an out-of-network provider for major health insurance companies. We strongly recommend consulting with your insurance provider before visiting us. Check if your coverage includes out-of-network health services to ensure minimal out-of-pocket costs for your treatment.

If you decide to proceed with our services, please accomplish the Assignment of Benefits form to authorize insurance payments to BIOKINETIC and acknowledge your financial responsibility for any uncovered charges.
